The form contains a medical declaration, veterinary (& mobility) declaration, and statutory declaration.

These declarations will need to be printed and completed in hard copy. 

 

You will also gain access to the online declaration for our team to complete.

It is here that you will also need to complete some information about your assistance dog and yourself, helping us ensure we have the most up-to-date information.

You will also be accepting the invitation to renew your associate membership with RAD.

Make sure to sign the online form and submit it to us.

 

Once we have everything, we will have your new membership ID card printed.

Once this is done, we will reach out to you to return your expiring ID, and we will send your new one to you via express post.

 

Please make sure to have this done on time, to help ensure we can have your new ID out to you promptly.
